Hamilton Mobile Chiropractic uses this website to share general business information and receive appointment requests. This page explains the limited information the site collects and how it is used.

Information you choose to send

The scheduling form may collect your name, phone number, email address, ZIP code, preferred timing, service of interest and a short scheduling note. We use that information to respond to your request and coordinate an appointment.

Do not submit medical records, diagnoses, insurance numbers or other sensitive health information through this public website form. The form is not a patient portal.

How information is handled

Form submissions are sent to a business-controlled Google Workspace workflow. Information may be accessed by authorized people or service providers who help respond to scheduling requests and operate the business. We do not sell website lead information.

Calls, texts and email

When you submit the form, you consent to a call, text or email about your request. Message and data rates may apply. You may ask us to stop nonessential messages at any time.

Website operations

The site may keep short-lived technical logs needed for security, reliability and troubleshooting. The public site is designed without advertising trackers or heavy third-party analytics by default.

Google Maps reviews

When live reviews are enabled, this website requests current public review data through a Google Apps Script service. Reviewer names, photos, profile links, ratings and review text are supplied by Google Maps. The website does not persist Places review content. Loading reviewer photos or following Google links sends technical request information to Google. See the Google Privacy Policy and our Terms.

Medical privacy

This public marketing website is not intended to collect protected health information. Any privacy notices or consent documents that apply once you become a patient will be provided separately through the appropriate clinical process.
